Hives, eczema and skin that will not stop itching

Skin is the most visible thing about a complaint and often the least of it. A consultation here will spend a surprising amount of time on things that are not your skin, and this article is about why.

The skin is read as an outside edge

TCM treats the skin as continuous with what is happening inside rather than as a surface to be worked on separately. So a consultation about a rash asks about digestion, sleep, stress and, where relevant, the menstrual cycle — not as small talk, but because in this framework they belong to the same picture.

The physician will also want the practical history: how long it has been going on, what makes it better or worse, what you have already tried, and what you are putting on it now.

The words a physician will use

Wind, heat, damp, blood dryness. These are pattern descriptions — ways of grouping what has been observed so that a treatment plan follows from it. Wind in TCM describes symptoms that move and come and go; it is not a reference to an allergen, and being told there is wind is not being told you have an allergy. Only allergy testing settles that.

Where a doctor's diagnosis fits

Eczema, urticaria and contact dermatitis are medical diagnoses, made by a doctor and often supported by patch or allergy testing. Psoriasis is different again. TCM does not make these diagnoses and cannot confirm or rule one out.

If you have been prescribed a cream or a tablet for your skin, bring the details, and do not stop using it because you have started TCM treatment. Stopping a prescribed steroid abruptly can make skin worse than it was. That is a conversation with the doctor who prescribed it.

What a consultation involves

Conversation, a look at the affected skin, and pulse and tongue examination. Treatment is usually herbal medicine, sometimes with acupuncture, and is reviewed at each visit against what you report about the days in between.

When to see a doctor

Go to a doctor now, not later, if a rash is spreading quickly, blistering or weeping, comes with a fever, or if you have any swelling of the lips, tongue or throat or difficulty breathing — that last combination is an emergency. See a doctor too about any mole that has changed. None of this is a reason to stop TCM treatment. It is a reason to have both.
